terms and conditions:

We will ship your knee CPM and knee rehab machine request the following:

• Customer to rent for a minimum of 4-6 weeks
• The customer pays shipping; we will provide the new sterile pads and knee machine
• Damage to the knee CPM machine while in use will be taken from a $600 deposit
• The customer must return CPM in good shape and will get the $600 deposit back
• Contract to be provided to be signed prior to shipping of the Knee CPM
• If under a doctor's care, your doctor must provide written consent for Knee CPM use
